Use this form to register or renew lessons. You can register one student per form.  To register additional students, use the link at the end of the form to "submit another form."

RETURNED PAYMENT AND LATE PAYMENT FEES
By completing this form, you understand and agree that failure to pay student lesson fees or any monies due and owing Seton Hill University by the first lesson of the new lesson set, Seton Hill University may assess a late payment fee at the rate of $15 per month until past due account is paid in full.  If a payment made to the student lesson account is returned by the bank for any reason, you agree to repay the original amount of the payment plus a returned payment fee of $50.

REFUND POLICY:  For private lessons, no refunds will be made after the fourth lesson.  Before the fourth lesson of any session, paid tuition will be refunded for any unused lessons. After the fourth lesson, students are responsible for the full fee. Collections agency fees: You understand and accept that if Seton Hill University refers your student lesson account balance to a third party for collection, a collection fee will be assessed and will be due in full at the time of the referral to the third party.
