Summer Speech Boot Camp Questionnaire
The Summer Speech Boot Camp at AutismUp is a 6-week long collaborative speech program developed for individuals 16+ years old who are nonverbal or have limited verbal skills. Participants should already use or be interested in trialing an Augmentative Alternative Communication device. Qualified participants will be paired one-on-one with a graduate student from Nazareth College's Speech-Language Pathology program and supervised by a licensed and ASHA certified speech-language pathologist. The program will be broken up into three intensive hour-long blocks: individually-focused teaching, group practice, and individual review and skill reinforcement. At the end of the program, participants will be able to leave with greater AAC skills, an update to their current system, or a recommendation for a long-term system that can be implemented with their speech-language pathologist.

Summer Speech Boot Camp Enrollment Process:
1.) Complete this AutismUp Summer Speech Boot Camp Questionnaire using the following google form. To begin form, click next at the bottom of the page.

2.) All prospective camper applications will be reviewed and pre-screened to determine eligibility for the AutismUp Summer Speech Boot Camp.  An AutismUp Staff or Nazareth Facilitator will confirm camper eligibility and contact you directly to complete enrollment and schedule an intake meeting.  If your camper does not meet the criteria for the Speech Boot Camp program, we will contact you to discuss alternate programming options available through AutismUp.  

3.) If all criteria is met for enrollment, AutismUp staff will work with you to enroll your camper in the desired camp group, session(s), review payment options and set up payment.  Note:  If you have not previously created an AutismUp Family Profile, an AutismUp Staff will instruct you to fill out the online form in order to complete registration for camp.
