Who are Catalyst Foundation and Multicultural Aged Care?
Catalyst Foundation is a not-for-profit organisation and the leading provider of community information in South Australia. We connect people to services and we provide free and independent information to all South Australians.
Multicultural Aged Care (MAC) supports CALD-specific and generalist agencies to meet the Aged Care Quality Standards by delivering exceptional services to the CALD community.
As a consortium, Catalyst Foundation and MAC provide the Care Finder Program across metropolitan Adelaide

What is the Care Finder Program?
The Care Finder Program provides specialist and intensive support to specifically targeted people who have one or more reasons for requiring intensive support to interact with My Aged Care, to access aged care services and/or other relevant community support in the Adelaide geographic region.

Who can be referred to the Care Finder Program?
Senior Australians who are eligible for aged care services and have one or more reasons for requiring intensive support to either:
  • Interact with My Aged Care (either through the website, contact centre or face-to-face Services Australia service centres) and access aged care services and/or
  • Access other relevant supports in the community.  
Individuals who are eligible for aged care services includes those who:
  • Need help with one or more everyday tasks and be aged;
  • Aged 65 year or older (50 years or older for Aboriginal or Torres Strait Islander people).
  • Aged 50 years or older (45 years or older for Aboriginal or Torres Strait Islander people) on a low income and homeless OR on a low income and at risk of being homeless.  
The Care finder Program supports people living in the following local government areas:
Adelaide, Campbelltown, Charles Sturt, Marion, Norwood Payneham and St Peters, Onkaparinga, Playford, Port Adelaide Enfield, Prospect, Salisbury, Tea Tree Gully, Unley, Walkerville, West Torrens.

How can the Care Finder Program assist clients?
The Care finder Program assists eligible vulnerable persons to connect to Aged Care services, health systems and community services or programs.  
Care finder provides assistance which includes:
  • Support clients to interact with My Aged Care so they can be screened for eligibility and referred for assessment.
  • Explain and guide clients through the assessment process
  • Assist clients to find the aged care supports and services they need and connect with other relevant supports in the community.
  • Provide support while a client is waiting for an assessment or for services to commence.
Care finders check in with clients so that:
  • They understand the next steps and where they are in the process.
  • Whether they need support to remain engaged in accessing services.
  • Whether their needs have changed.
  • Whether they have any need for support from the care finder.    
Care Finder Program Eligibility Criteria
Care finders specifically help vulnerable older people who need intensive assistance to access aged care and other supports.
To receive care finder support, a person must:
  • not have a carer or support person who can help them, or
  • not have a carer or support person they feel comfortable with or trust to support them, and 
  • be eligible for govt funded aged care services
