Agreement of Use and Liability Form.

The student has read and understands the STEM Student Success Center Laptop Loan Program description and the LACCD Use of District and College Computing Facilities policy (https://www.laccd.edu/About/Documents/AdministrativeRegulations/B-27.pdf) as it pertains to the loan of a West LA College laptop computer.

As stated in the LACCD policy section IV, the student agrees not to perform any malicious activities that would violate District policy with respect to the usage of the laptop computer including but not limited to hacking, downloading or uploading pirated content or software, circumventing installed District software, or using the laptop in a manner that violates state or federal laws. Misuse of the laptop may result in the loss of participation in the loan program.

The student agrees to accept responsibility for the loss or damage to the laptop and any of its components during the load period. Theft, loss or damage to the laptop must be reported to the STEM Student Success Center immediately but no later than the next business day. Failure to follow this policy may result in financial liability for reimbursement or repairs of the laptop.

The laptop must be returned by June 30, 2024, with the option to request a renewal for the next academic year. Any laptop not returned within 2 days of the return date will be considered late and recovery efforts will be made. If the laptop is not recovered within 5 days of the return date, the laptop will be considered missing, and the borrower could be financially responsible for replacement.

All personal data must be removed before returning the laptop. Returned laptops will be reformatted and all remaining data will be deleted. STEM Student Success Center is not responsible for any loss of personal data.
