I give my consent for my child\ren's photograph to be taken for use in school publications and promotional materials e.g. newsletter, school magazine, parent information books/brochures, School Management Systems, class blogs, social media and school website and Intranet site. I understand that all photos are stored on Department of Education approved systems.

In the case of an outbreak of headlice, I give permission for my child\ren's hair to be checked by the school-authorised officer. I understand that it is my responsibility to inform the school if my child has headlice and that he/she can only return to school after treatment has been applied.

From time to time Learning Communities may have the opportunity to watch movies with a PG rating during school hours. I give permission for my child/children to watch PG rated movies.

I give permission for the school to contact me via email (email addresses, like all other parent information, is always treated confidentially by the school and will never be shared or disclosed to another party without parent permission). If your email changes, please notify the school immediately.

An essential part of children's learning, the internet is a tool which children will have experiences with at school.

When working on the Internet, children will: only work on the web for purposes specified by the teacher; not give out information such as surname, address, telephone number or parent's work address/telephone number;  never send a person their picture without first checking with their teacher; always have the teachers' permission before sending an email; compose email messages using only language acceptable at school; not respond to any messages that are unpleasant or that make them feel uncomfortable in any way; will not use material from other websites unless they have permission from the person who created the material; not use the Internet to frighten or harass other people; follow school guidelines and procedures when preparing materials for publication on the web.

I have read these conditions to my child\ren and understand that breaches of such will result in my child\ren losing their internet/email access rights for a period of time determined by the teacher and Principal.

Our school provides access to online services. These increase the range of teaching tools available to staff and enhance the learning opportunities available to students. To access these online services, we will need to create accounts on  student's behalf. This will involve the school using the student’s full name, home group and year level to access their unique online services account. Do you give Armstrong Creek School permission to create online services account?

I give my permission for my child\ren to participate in local incursions/excursions (in school and within walking distance) and sporting programs within walking distance. I understand that the school will communicate with me prior to these events, as a reminder.
