This survey is for parents who would like to register their intent to send their child/children to school during Week 5, Term 3 - Monday  August 10th to  Friday August 14th.  The registration for Onsite Learning needs to be done by Friday 12pm.

Onsite Supervision – you can only request on-site supervision if you cannot work from home and no other arrangements can be made.  

We may need to reduce the numbers of both students and staff onsite and if so it may be necessary to allocate days to some families. Sacred Heart School will help support families with on-site supervision and provide a skeleton staff each day to supervise children where there is no adult at home. All students, whether they are on the school site or learning at home, will have the same work provided for them.

PLEASE ONLY FILL IN THIS FORM IF YOUR CHILD / CHILDREN ARE UNABLE TO LEARN FROM HOME IN WEEK 4 AND ARE ELIGIBLE ACCORDING TO THE ABOVE CRITERIA.  THANK YOU

We are offering normal times for BSC (from 7:45am) & ASC (to 5:30pm) for families of essential workers for this week only (Week 5 from Monday to Friday).
