SECTION 4 - Product Entry Information: Enter your product name and competition category, remembering that products are often eligible for more than one category.
SECTION 5 - Additional Product Entries: If you would like to enter more than one product or category, add your product names and competition categories here.
SECTION 6 - Payment: Please enter the email address you would like us to invoice. Invoices may be paid by credit card or ACH.
SECTION 7 - Read the 2023 Rules and Regulations. By checking the "I agree" box and submitting this form, you agree to the 2023 Rules and Regulations.
To complete your submission - Mail TWO (2) product samples PER ENTRY to the address below. For example, if you are entering a bloody mary mix into the "New Product" and "Bloody Mary Mix - Mild" categories, you would need to send four total product samples.
