I hereby register my child for the above described camp and authorize the staff to direct them in participation in camp activities. My child has no medical or emotional problem which may effect their ability to safely participate in the program. The camp is authorized to respond to any health problem or injury they may incur while attending camp. I understand that my child must have current and active medical insurance before they can attend camp. Neither I nor my child will hold Sports Beyond, its staff, or Marvin Williams Center liable for any injuries or expense related injuries while my child is at camp.
