This sign up sheet is a request to be enrolled in an upcoming Quality Matters online 18 day APPQMR course. There are several dates to choose from and space is limited. YOU MUST CANCEL WITHIN 72 HOURS OR THREE DAYS PRIOR TO THE FIRST DAY OF THE COURSE.  There will be an information session for this course held on Wednesday the week prior to the course start date which will outline the expectations and an overview of how this course is managed.
