The threefold purpose of this form is to Convey the Details, Connect to the Heart, and Creatively Deliver. Please fill out this form as early as possible (preferably 3 months out). Completing this form later may jeopardize our ability to fit it into the calendar and plan for proper promotion. Details can be adjusted in the future by contacting the church office.
