Please complete this form EVEN though you will have completed a form last week. There was an error with the form initially and then a number of parents completed it multiple times meaning the spreadsheet is now huge!

This will be the ONLY form you will need to complete if you need set days each week.

However, if you are on a rota/shift pattern etc and need different days each week, please let us know on THIS FORM and we will contact you each week (by Thursday) to ask about the week ahead. On this form, please let us know what you need for week beginning 11th January and we will contact you each week (by Thursday) for your days for the following week.

If you are requesting a NEW space - please be patient and avoid sending a MyEd/emailing and phoning as well as this holds up the process.  ALL spaces are offered through THIS form, but at really busy times it could take 24hrs before someone gets back to you.

We have had a significantly high number of requests and some classes are now full. For these classes, we may have to ask for proof of your key worker role and if needed, may have to only offer spaces where both parents are key workers. We may also have to offer you a space in a different class for the duration as bubble mixing is not allowed.
It is essential that you do not send children to school if they can be at home - the safety of the children and the staff at this time is our absolute priority.

If you have one key worker in the family but another parent at home, please do not request a space at this time.
