CCPS Families & Staff–
If the child or adult is  fully vaccinated or 11 years or younger use 5*days
If the child or adult  is 12 or older and not fully vaccinated  or immune compromised use 10* days

In the last (5, 10)* days  answer the following questions.  Your answers pertain to all names and any people the child or staff member is living with.   If you answer yes to any one of them, you or your child/ren may not attend the program today. Please contact Community Cooperative Preschool at 613-224-6173 to discuss any further action. An email stating the reason for the absence must also be sent to director@communitycoop.ca.
