State FCCLA Fashion Show Application 2023
Complete the following information with as much detail as you can. 

Guidelines

  • Fashion Show will be held during the awards assembly on March 21, 2023.

  • All entries must be a STAR Event participant in either Fashion Construction, Fashion Design, or Repurpose/Redesign at the 2023 Utah FCCLA State Leadership Conference.

  • Only the participants (and models) are allowed in the setup area. Other persons may not assist. Participants must bring all necessary supplies and/or equipment to preparation time.

  • Students will need to attend a fashion show practice. More details will come from Becky Sagers rsagers@wsd.net.

  • Students who submit this form agree that the garment will be an expression of your own ideas and work. You further give permission for Utah FCCLA to display my design during the FCCLA Fashion Show, on the FCCLA Website, and in photographs used for promotional purposes.

  • By submitting this form your adviser and parents give permission for this student to participate in the Utah FCCLA Fashion Show at this year's State Leadership Conference. You, your parent, and your adviser have discussed this and certify that the student has read and understands all guidelines of this show and that he/she will abide by them.
