**IMPORTANT INFORMATION - PLEASE READ BEFORE COMPLETING REGISTRATION FORM**

This form is to register for our WEST ISLAND camp located at 52 Rue Joseph Paiement, Dollard-des-Ormeaux, QC H9A 1J1.

This form will take 15-20 minutes to complete. All information in this application will be kept confidential within the Camp Cosmos team.

For each child you wish to register you must complete a separate form. Campers must be at least 5 years old and have completed kindergarten, and the maximum age is 11 years old.

Spaces at Camp Cosmos are allotted on a first-come, first-served basis. Please note that YOUR CHILD'S PLACE AT CAMP WILL NOT BE RESERVED UNTIL YOUR REGISTRATION INFORMATION IS COMPLETE AND YOUR DEPOSIT HAS BEEN PAID  The amount of the deposit is equivalent to double your weekly camp fees and will be held until the final two weeks of camp. If there are no outstanding charges, the deposit will be applied toward the camp fees at that time. Upon completing this form you will receive an email containing detailed instructions on how to pay the deposit and complete your registration.
