How do you ask for consent?
In order to support prevention programming, the Title IX Office in consultation with the Coordinated Community Review Team (CCRT) is collecting anonymous student feedback to better understand practices related to consent. Please keep in mind, all of your answers are ANONYMOUS, and no one from the CCRT will see who you are or reach out to you for more information.

Student feedback on this survey may be used to support future sexual violence and sexual harassment prevention efforts. Students can also share what types of prevention training they would like to see going forward.

Before you begin, we want to let you know that some of these questions may be difficult for survivors of sexual violence and sexual harassment. You can skip any and all questions. We will provide content advisories throughout to indicate topics that may be particularly difficult for survivors. If this survey elicits difficult feelings because of a past experience of sexual or interpersonal violence, you can get support from the confidential CARE office at care@ucsc.edu or 831-502-2273.

The survey contains 'yes/no/other' questions in regards to specific sexual activities. It is not intended to direct people towards a "correct" answer, but rather to provide an opportunity for students to share their current consent practices, which may be useful in informing future prevention training.

This survey should only take about 5-10 minutes.
