This Nomination Form must be submitted by March 1, 2024

Distinguished Alumni Award Criteria
Nominees for this award must meet the following criteria:
  • The nominee must have graduated from Sandy Valley High School with a minimum of 10 years prior to the year in which the award will be presented.
  • The nominee must have demonstrated recognized achievement in his/her profession areas of business, industry, the military, education, medicine, non-profit service organizations, the arts, community service, government, and charitable endeavors.
  • Nominees must be presented to the selection committee via a completed nominee application package. An alumnus may not nominate himself/herself but the nomination can be made by members of his/her family, fellow alumnus, or any other individual with knowledge of that individual’s accomplishments.
  • Posthumous nominations are accepted.
