Emergency School Closure Plan

Please fill out a separate form for each child. Please reply to this form by Monday December 6th.

Occasionally an emergency arises that requires school to close unexpectedly. In this event, announcements will be made on all local radio and television stations. To plan for this possibility, you will need to make arrangements for where your children are to go if dismissed early. Please review this procedure with your children to make sure they understand and are not confused about what to do. Please understand that with limited phone lines, it would be impossible to have each child call home.

I've instructed my child that if school is dismissed early due to weather/emergency, I want him/her to: Select ONLY ONE option, 1 or 2

Option 1- Go Home as Usual

Go home as usual (bus/walk/picked up/daycare/babysitter). In the event of an emergency closure please note that Champions would be canceled. Please contact the office to make transportation arrangements.

Option 2- Be released to the following people as pre-arranged by me; I understand that emergency contacts on my child's student info may also be called. (Please include full name and phone number)

I have discussed these options with my child. I understand children may not phone home for a ride or remain in the building, since all school staff may also leave early.
