YOU MUST BE REGISTERED FOR THE COURSE IN ORDER TO SUBMIT THIS REQUEST. The Course Must already be on your registration.
All forms are distributed during regular business hours Monday - Friday 9 am-5 pm. Distribution may take up to 2 business days. Please check your Junk & Spam for "DocuSign" to double-check to see if your form has been dispatched.

1. Once a certification form is completed with all signatures, (Student, Advisor,  the form will be automatically sent to the Host institution and a Completed copy will be emailed to you.

2. If you do not receive a Completed email please check your progress by reopening the first link sent to you for your request. This will allow you to see who has signed and where your form is in the process.
No Official CAU business will be conducted via the Gmail accounts linked to DocuSign.
