This form is for ALL families of students (3rd-5th grade) who would like their child to participate in State Testing this school year. By completing this form, you are registering your child to participate in State Testing (ELA, Math, Science). If you do not complete this form, your child will not participate in State Testing this school year. Any student who does not participate in the 2021 NYS ELA, Math, or Science exams will not be penalized. If you choose to complete this form, you will need to bring your child to school to be tested on select days in April, May, and June. Al completar este formulario, está registrando a su hijo/a en los exámenes estatales (artes del lenguaje inglés, matemáticas, ciencias). Este formulario es solo para padres de estudiantes remotos (3º a 5º grado) que deseen que su hijo/a participe en los exámenes estatales de este año escolar. Si opta por completar este formulario, deberá llevar a su hijo/a a la escuela para que tomen los exámenes en determinados días de abril, mayo y junio. Si tiene más hijos en el 3º a 5º grado, complete este formulario varias veces, una por cada niño/a que le gustaría que tome los exámenes.
