As part of New York State's commitment to digital equity for all, school districts are now required to collect individual student data as it pertains to the students' access to technology at home.  To assist the district in providing the State with this information, we ask that you  please complete a separate form for each student in your household.  All information collected in this survey will be kept confidential.
