Girls 2 DIVAS Summer Camp Application 
Transformation camp is a free summer camp for 3rd-12th graders. Lunch provided. Drop off at 7:30a. Pick up at 5:30p.  
Please complete the application below. One applicant per child. (Please note, space is limited, once we reach 25 applicants, the registration will close. You will receive an email and call by next week Wednesday

Once selected, We are hosting a mandatory meeting in person on June 3rd from 1:30-2:30p. Will you or another parent or guardian be able to attend? There is paperwork that must be completed before your Diva can start camp. *
Terms:
-Camp ends at 5:30p. After 15 min. a late fee of $10 will be applied.
-Camp is from June 5th-July 28th
-Camp will not be held on 4th of July
-Parents are expected to download the Groupme application on their phones to communicate with camp staff
-Diva will attend 85% of camp (per our contract with grant)
-We are providing Lunch. The parent/guardian will provide breakfast and snacks for DIVA.
