Change of Contact or Billing Information
In order to update your contact information, please enter your child's student ID number (found in your Jupiter account in the format of A000####) and update the necessary data fields. Please note that for child protection purposes, if one parent's information is removed, it is considered a change of custody (divorce or death), so please also submit the legal documents to the office along with this form. 情報更新は、お子さまの学籍番号（ジュピターのアカウントにあるA000####）を入力し、更新必要な情報箇所に英語で必ずご記入下さい。なお、生徒保護の観点から、片親の情報削除は、親権者の変更（離婚や逝去）とみなされますので、このフォームと共に法的書類も事務所へご提出下さい。
