CYCC Champion Award Nominations
The Champion Award is given to youth (under age 21) with chronic conditions who have shown extraordinary leadership in and/or commitment to their school and community.

Please help us by nominating the young person who comes to mind. The process is simple: Fill out this Nomination Form below, telling us about them and how their efforts and/or leadership have made an impact in their community.

Awards will be presented at CYCC's Annual Kids Chronic Health Awareness Event, hosted at the McAuliffe-Shepard Discovery Center on Thursday, July 11th from 5:00 to 7pm. Winners will be announced before the event, so you can plan for attending the event.

The deadline for nominations is May 31st.
