Scouts, Scout Leaders, or ANYONE ATTENDING a scouting activity must complete this COVID-19 screening less than 30 minutes prior to participating.

Please Note:

1) All questions are required. Failure to answer every question will prevent submission of the form and will result in the individual being prohibited from participating in the activity.

2) If any responses are “YES”, temperature reading is 100.0 Fahrenheit or higher, or symptoms are reported, the individual will NOT be allowed to participate.

3) Anyone who becomes sick or develop symptoms will be isolated from the other Scouts, a Parent/Guardian will be notified, and the Scout must leave the scouting activity as soon as possible.  This is in compliance with CDC and BSA Guidelines.
