Please complete this form in its entirety.  Campus-based troubleshooting will take place within 48 hours of the request or earlier.  This form must be completed and submissions will be answered in the order in which they are received.  

Please note that depending on the nature of your request, some repairs will have to be completed by our Tech Services Department representative at 281-498-8110 extension 22530.

 If this is the case, you will be notified by email. Please make sure you have reviewed the "Who do I contact" section in Schoology prior to submitting a request so that you are contacting the appropriate person and your request(s) are handled in a timely manner.
