We take responsibility for our family's health & hereby expressly waive and release any and all claims, now known or hereafter known, against Alpha Acton Academy Folsom and any related guides, directors, employees, agents, affiliates, contractors, representatives, shareholders, successors and assigns (collectively, “Releasees”), on account of injury, illness, disability, death or property damage arising out of or attributable to the Participant’s presence on the Premises or engaging in any Activity and being exposed to or contracting Covid-19 or any other Illness, whether arising out of the negligence of the Directors any Releasees or otherwise. The Participant covenants not to make or bring any such claim against the Academy, the Company, The Bridge Church or any other Releasee, and forever releases and discharges the Founders, Directors, the Company and all other Releasees from liability under such claims.

I consent to my child(ren), my spouse, and myself being photographed or videotaped for use by Alpha Acton Academy Folsom. I recognize that media will be used at the discretion of Alpha Acton Academy Folsom, but only to further share the vision and mission, and not for other personal or commercial gain. I also recognize that Alpha Acton Academy Folsom is committed to the safety and well-being of my child(ren) as their first priority. Examples of media use are: weekly blog for parents and families about each events and goings on, media about the school for interested families, social media posts.
