Digital Survey Consent Form
First of all, I would like to thank you for your participation with this survey.

The purpose of this study is to discover possible inequities faced by adolescents who identify as a sexual minority compartive to individuals that identify as heterosexual. Research will focus around the mental health of participants and if applicable, identify commonalities between responses. This study will contribute to the researcher’s completion of his AP Capstone Diploma.

Please know that:

Your survey responses, along with the responses of other participants, will be used to help me complete a research study that seeks to identify possible mental health inequities in those that identify as a sexual minority.  

The interviewer and researcher, Kash Calvin, is an AP Research student enrolled in AP Capstone program at Haas Hall Academy Bentonville.

Your participation in this survey is completely voluntary.  You may choose to stop the survey at any time if you feel uncomfortable, and there will be no penalty whatsoever for choosing to do so.  You may choose to skip any questions you do not want to answer; however, it would be helpful for you to answer as many questions as you are able and willing to answer.

All responses are confidential.  Your identity is only known to me, and no information directly associating your name with your responses will be reported to anyone.  If specific information from your survey that pertains to you individually is used in the report of findings, I will make sure to identify each person by using a pseudonym or a randomly selected number when analyzing and reporting on my findings.  Typically, findings are reported in group form, so no individual is identifiable.

There are no direct benefits to you anticipated from your participation; however, it is possible that the information you provide in the study will increase recognition over the niche topic of sexual minority mental health. The only risk you may encounter in this study is emotional stress. However, the survey section pertaining to mental health will have a disclaimer before you begin. You may choose to end the survey at any time. There may be uncommon or previously unforeseen risks. You should report any problems to the researcher.

The personally identifying data collected from this survey will be destroyed upon the publication of the researcher’s academic paper.
