AHS Volleyball Uniform Order Form-Due June 10 by 11:00am.
Please fill out all item you need to order below.  I will not be ordering extras, so if you need something, let me know that on this form.  

Order only the items you need.  I need everyone's jersey size and warm up/practice shirt size because those have to be ordered.  All other items are "if needed".   If you need game shorts, knee pads, or shoes, order those as well. If you already have those, no need to order more.  THERE WILL BE NO NIKE PRO SHORTS WORN TO GAMES.  IF YOU ONLY HAVE NIKE PRO GAME SHORTS, YOU NEED TO ORDER DIFFERENT ONES.  NO EXCEPTIONS!!!

$ for ordered items is due by July 5th practice date.  Let me know if there is an issue with that.

ASO Ankle Braces-I STRONGLY, STRONGLY,  recommend that you all order 2 of these--one for each ankle.  It is not required, and if you already have a brace or do not choose to wear them, you do not have to order.  $25 each brace ($50 for pair)

Game Shorts-Choose Compression or Alpha Echo (Only order if needed--if you have some, no need to order more)  Must be black and CANNOT BE NIKE PRO.  Can order both if you want...to order both, select both boxes here and enter size in the next question.  Either of these shorts can be worn as game shorts.

Knee Pads (LR6 Mizuno)-$30--We will wear white kneepads for games.  If you already have some, no need to order more.

Mizuno Shoe (Wave Lightening 6)-$100    If you have some or find different court shoes you want...that is fine..try to keep them gray, black, or white or a combination of those colors.
