Participation Form for One (1) Device UUCA Congregational Meeting via Zoom April 11, 2021
Please use this Participation Form for only ONE (1) DEVICE: computer, tablet, smart phone or phone.
     * If 2 or more members are in the same household and will use the SAME DEVICE, these
        names should appear on THIS PARTICIPATION FORM in #1 and #4.
     * If 2 or more members are in the same household and will use DIFFERENT DEVICES,
        each member should fill out a SEPARATE PARTICIPATION FORM.
For the upcoming zoom Congregational Meeting, please complete and submit this Participation Form so we can pre-register those planning to attend and vote. Voting members are those with a pledge of record who have been a member for at least 30 days prior to the congregational meeting. If you have been a pledging member and do not have a current pledge of record, you will be asked to submit a pledge to the church that would enable you to vote at this meeting.

Please NOTE: If more than one member will attend the April 11th meeting at the same place, LIST THEIR NAMES HERE ONLY IF THEY WILL BE SHARING ONE (1) DEVICE. If each member will use different devices, each should fill out a DIFFERENT Participation Form.
6. If the response to #4 is yes, list their EMAIL ADDRESSES. If the response to #4 is “no,” go to Item 7. Please NOTE: If more than one member will attend the April 11th meeting at the same place, LIST THEIR EMAILS HERE ONLY IF THEY WILL BE SHARING ONE (1) DEVICE. If each member will use different devices, each should fill out a DIFFERENT Participation Form.
