Confirmation Worship Note Online Form
Please complete this form after you attend/watch worship. Confirmation youth are expected to complete 15 Worship Notes each year. Of these 15 Worship notes, we ask that 6 be Family Worship Notes and at least 3 be completed during Lent (see below for more info). You are welcome to watch/attend up to 3 services a year at other churches (besides LOC) - just indicate which church it was in the form below.

*6 Family Notes - where you discuss worship with your family (but you can complete any of the online forms and just make note that it was a "Family Worship Note"). All can be family notes if you'd like.
*3 Lent Worship Notes (can include Holy Week services) - can be done as a family!
