INFORMED CONSENT FORM - You are being asked to participate in a study investigating your perceptions of participating in the 2Serve CERT disaster training. If you agree to participate, you can take a computerized online survey. You will also have the opportunity to participate in a debrief session which will be recorded. You will not be putting your name on anything. The information collected is for research purposes and any information you give us will be kept totally confidential. The online survey should only take 10 – 15 minutes. By checking "Yes," you are agreeing to participate and are giving permission for your photo to be taken. There are minimal risks associated with this study. If at any time during this study you wish to withdraw your participation, you are free to do so without prejudice.
