Completion of this registration form implies consent to the use of photographs, video and sound recordings of your child for staff training and quality assurance as well as marketing and promotional purposes in print, media and online. If you do not consent, please call 2547 9114 before completing this form.Applications will be taken on a first-come, first-served basis, and will only be accepted on receipt of payment. Confirmation of acceptance will be sent by email within 14 days of receipt of this form. Requests to withdraw your child should be made in writing up to 14 days before the workshops start and will be subject to an administration fee of HK$600. Cancellations after this date cannot be refunded unless your child has incurred injury/illness (with doctor’s note) preventing them participating.No refunds or credit notes can be made once workshops start. Due to the nature of our workshops it is not possible to offer make-up classes.All classes will be cancelled in the event of a Typhoon Signal No.8 or Black Rain Signal if hoisted at 7am (morning workshops) or 12noon (afternoon workshops) and 2pm for evening workshops.
