This form, once completed, will serve as an attestation statement from the parent/guardian of a student who has been in isolation or quarantine, declaring the student has met all applicable criteria for release prior to the student returning on-site to attend school or any activity on school property.
Providing a false statement or failure to disclose prolonged symptoms that may result in other students or staff being exposed to COVID-19 may be reported to the proper enforcement authorities including, but not limited to, the Arkansas Department of Health (ADH).
