Complete this form to select the type of booth your organization requests for the 2022 MISD Back-to-School Bash on Saturday, August 6, 2022. Booth space is limited and will be filled on a first come, first served basis.

All vendor booths are required to provide a fun engaging activity during the event.

We also encourage organizations to decorate their exhibit area; however, all materials and displays must follow MISD school policy, be age appropriate and family friendly. Since this is a school district event, we ask that faith-based groups not use booths to promote their religious beliefs.

To ensure that MISD allocates sufficient resources, please submit your form and payment no later than July 30, 2022.

Option 1: Basic Community Vendor Booth (Cost-$300)

Option 2: Healthcare-Direct Service Provider (providing free haircuts, immunizations, sports physicals, or eye exams to students DURING the event) (Cost- Free)

Option 3: Event Sponsorship (Cost-$1,000-$10,000) (Additional information will be sent from MISD).
