This form is to be completed by the recommending teacher or instructor to nominate up to 10 students for the OCDA CHC. Students should currently be in grades 4-8 with unchanged voices. In order for the singers to be prepared for the June 26 event, we are asking teachers to help facilitate practices in May. In addition, practice tracks will be available to all singers and teachers. (We are not holding satellite rehearsals this year)
