This form offers a method to anonymously communicate requests, insights or incidents in the space to the Managers of the Brown Design Workshop. This includes (but is not limited to) commentary on tools, programs, hours, knowledge, culture and equity/inclusion-related incidents (sexual assault, harassment, or anything has felt wrong/uncomfortable in the space).

Note: In reporting equity/inclusion-related incidents, if you choose to include names, the Brown Design Workshop is obligated to follow up with those named and actively work to resolve the situation. If you choose not to include names, the Brown Design Workshop will work to address the incident as a general cultural issue.
