New Patient Questionnaire
Thank you for contacting Transcendent Psychiatry.

All prospective new patients will be asked to complete this questionnaire: it provides an opportunity to learn more about you and to see if Transcendent Psychiatry can assist with your treatment goals. This form is encrypted and compliant with HIPAA to ensure patient privacy.

Because Transcendent Psychiatry is a small solo outpatient telehealth private practice focusing on medication management for adults 18 years old and over, a limited number of new patients are accepted based on fit with current available treatment strategies.

With that in mind, the answers to this questionnaire will be reviewed. After, you will be contacted to either: 
- Schedule an initial appointment with psychiatrist Dr. Tran-Boynes 
or 
- Be provided with referrals if another practice is able to better address your treatment goals

The questionnaire must be completed by the prospective new patient only. Forms completed by anyone other than the prospective new patient will not be reviewed.

If the office is closed for an extended period (e.g. during the holidays), you will receive a notification email, and your questionnaire will be reviewed once the office reopens.

Insurance:
- At this time, Transcendent Psychiatry is in-network with 1) Cigna and 2) Blue Cross Blue Shield, including CareFirst and the Federal Employee Program (FEP)
- Transcendent Psychiatry is not currently accepting patients with Medicare and/or Medicaid 
- Lastly, for all other commercial insurance plans, Transcendent Psychiatry is out-of-network. Self-pay rates for out-of-network patients are $575 for a 75-minute initial evaluation and $250 for a 25-minute follow-up appointment. The frequency of follow-up appointments ranges from every 2 weeks to every 3 months
