player and/or her parent or guardian indicates that he/she has read this document and fully understands and agrees to its terms.

By signing this release and waiver, I agree on my own behalf and on behalf of my child that: Soccer is a physical activity. I/my child may, by participating, injure myself/himself/herself or trigger or aggravate a medical condition (known or unknown) which may be affected by playing soccer and participating in related activities.

In addition, damage to or loss of my/my child's personal property is also a possibility. 

I acknowledge and freely accept and assume such possibility and risk and voluntarily release and discharge that Iand/or my child have or may in future have against JPL Canada, event sponsors, event volunteers, owners and lessees of the event premises and facilities, and each of their respective predecessors, successors, affiliates, officers, directors, employees and
agents (collectively known as Event Organizers) from any and all actions, suits, demands and claims of whatever nature in law, in equity or pursuant to any statute, relating to or arising from any injuries, damage or loss to my person or property, however arising, that I and/or my child may suffer while participating in this event and any related activities. 

I also hereby give JPL Canada permission for Event Organizers to record any or all of my or my child's participation in this event for photos, motion pictures, TV, radio, recordings, videotapes and other media known and unknown, and to use same, no matter by
whom taken, in any manner for publicity, promotion, advertising, trade or commercial purposes without any expectation or right of reimbursements or compensation of any kind.
