Using this form means that you've read and agreed to the voiding rules in the TOS.

You must also DM the masterlist moderator @xxlerax or send a note to the @Ashuri_CS toyhouse user confirming that it was you that filled-out the form and not someone pretending to be you.

Simply a DM to @xxlerax or if you prefer toyhouse message @Ashuri_CS  titled "Voiding Ashuri" with a message saying you've requested to void your ashuri.
