This form is to be used for submitting a request for approval of an event that may require the need of a liability waiver. Events including, but not limited to, parties, sports, and field trips will require a University liability waiver and must be submitted for Office of Student Affairs approval
at least three (3) weeks in advance.
Failure to do so may delay or ultimately cancel your event.
Note: regular meetings or educational sessions are not required to be submitted for approval.
