This form should be used for requesting official and unofficial transcripts for college-related purposes such as:
     -- sending to colleges for admission, scholarship, or financial aid opportunities
     --scholarship applications, etc.

Please fill out this form completely, and allow a minimum of 7 days for transcript requests to be processed.

*Do not use this form if you are seeking an Official Records Request to transfer to another K-12 school. You will be able to request official school records / report cards by completing the withdrawal process.
