2024-2025 Carey Kindergarten Registration

This is for residents of Carey Exempted Village Schools only.

Your child must complete kindergarten screening in the district of residency.

If you live in another district and are considering open enrolling to Carey Schools, please contact the office at 419-396-7922 opt 2. The required open-enrollment application and information will be provided. You may also download the application from our website or pick up in the office entryway.

Please note the OE window does not open until March 1st thru April 15th. 

All prospective kindergarten students must be FIVE years old by August 1, 2024.  

Step #1 - Complete this Google Form

Step #2 - Link for appointment times -  https://ptcfast.com/schools/Carey_Elementary_School_1

Step #3 - You will need to bring with you, YOUR CHILD and the following items:  State issued 
birth certificate (not a mother's/hospital certificate), immunization records and proof of residency.  
Custody papers if applicable.

Step #4 - FINAL FORMS - Once the 2024-2025 School Year is open and ready, a link will be sent to 
the email entered on the Google Form. All parents are required to register on Final Forms prior to 
their scheduled screening appointment. With using Final Forms ALL Parents MUST have a valid e-mail address as paper forms are not an option.

Kindergarten Screening will be held on Friday, April 12th at Carey Schools, 2016 Blue Devil Drive.  

Please allow 1.5 - 2 hours for screening to be completed.

Parking will be available in the main lot (North side).  
We ask that you enter through Door #1.  
Screening will be held in the auxiliary gym.
