Semester 2 Credits/Service Hours/Attendance Issues Form

*PLEASE READ CAREFULLY BEFORE FILLING OUT THIS FORM*

Fill out this Google Form if you believe you are missing any credits, service hours, or attendance.

Please note the following:

      -  You MUST indicate the name of your event EXACTLY as seen on the cumulative spreadsheet. For example, if you sold a box of pretzels, you must indicate that this was a part of the Gourmet Pretzel Fundraiser.
      -  You MUST include the number of credits/service hours you are missing from the specific event. For example, you must indicate it is (EVENT- #).
      -  If you do not provide a DETAILED description of your contribution to the specific event, you will not receive credit. Please be as detailed as possible in the description section.
      -  Note that falsifying your events on this google form will result in SERIOUS disciplinary consequences. If you did not attend/participate in the event, DO NOT fill out this google form. The officer team will not hesitate to reach out to you.
      -  If the event's deadline did not approach yet, please wait to fill out this google form! Make sure to wait a week AFTER the event ENDS and proceed to fill out this form. For example, if you have completed the January educational video earlier on in the month, please wait till the end of the first week of February to fill out this form. Please be patient as the officers are working hard to accommodate each and every one of your credits/service hours into the spreadsheet.
      -  DO NOT fill out this google form multiple times, as we are looking into all of your responses. If it has been a while since your event had occurred, please contact Arjun Birthi (Secretary) or Srihitha Chakilam (President) directly.

Failure to do any of the points listed above will not receive credit.

*IMPORTANT NOTE: The cumulative spreadsheet IS glitching. If your credits are glitching out, DO NOT worry and check back again at a different time. The officer team is looking into it.

Please wait one week after the DEADLINE for an event for the credits/service hours to be updated. Ex: If you completed the January Educational Video, wait until the first week of February for the credits to be updated. Please be patient as we are trying to record your credits/service hours/attendance as accurately as possible.
