FPP ENROLLMENT FORM FOR SCHOOL YEAR 2021-2022
Please fill out one form for EACH child that you are enrolling!  The registration fee for the 2021-2022 school year is $90 and is non-refundable.   Once we receive your enrollment form, we will send an invoice for the registration fee of $90 plus one month's tuition which will be applied to May 2022.  UNTIL WE RECEIVE YOUR REGISTRATION/ENROLLMENT FEES YOU WILL NOT BE FULLY ENROLLED IN THE PROGRAM.  This invoice will come via email and can be paid by debit/credit card.

WE WILL BE OFFERING BOTH IN-PERSON AND VIRUTAL LEARNING FOR THE 2021-2022 SCHOOL YEAR!

Tuition for each class is as follows:

PRE-K (5 DAYS) - $450 per month  (Turning 4 by September 30, 2021)

MULTI-AGE "BUBBLE CLASS" (5 DAY) - $450 per month  **You will need to enroll in this class if you plan to use After Care!!**

MULTI-AGE (5 DAY) - $450 per month **You should choose this option if you do NOT need After Care**

MULTI-AGE (3 DAY) - $375 per month

The following documents are required to enroll.  PLEASE BRING ALL PAPERWORK TO FPP DURING THE WEEK PRIOR TO THE FIRST DAY OF SCHOOL TO ENSURE THAT YOUR CHILD IS READY TO LEARN ON THE FIRST DAY OF SCHOOL!

STATEMENT OF UNDERSTANDING RE: Online Learning  **Only required for virtual learning option**
I understand that an educational partner will need to be present when my child is participating in the online learning environment.  I further understand that this educational partner is not to engage in the class discussions, unless specifically asked by the teacher during the synchronous learning times.  I also understand that it is important that my child is ready to learn at the specified time.  Finally, I understand that during synchronous learning, it is not appropriate to have siblings or other members of the household participate in the class since this is time that my child has specifically with their classroom teacher and other classmates.
STATEMENT OF UNDERSTANDING RE: Registration and One Month Tuition *
I understand that by filling out this form, I consent to paying a $90 non-refundable registration fee and one month's tuition to secure a spot for my child.  I also understand that the tuition payment may be applied to the last month my child is enrolled in the program.

I understand that I am responsible for paying tuition on time each month and that all monthly tuition payments are due NO LATER THAN THE 10TH DAY OF THE MONTH for each month that my child is enrolled.  I further understand that if my payment is received past the 10th day of the month, I will be charged a $20 late fee.

I understand that FPP offers the option of paying by credit or debit card for my monthly tuition payments.  I further understand that if I choose to pay by debit or credit card, I will pay the credit card processing fee in addition to the tuition amount.  I also understand that I am able to pay in person each month by check or cash if I do not want to pay by credit/debit card.
