Class of 2022 Mandatory Graduation Form
Seniors,
Before we celebrate you during upcoming events such as senior recognition and graduation, we need the following:
 1. Additional information to include in the programs
 2. Selection of who you would hope to have present your diploma
 3. Indicate where to send your final transcript
 4. Feedback on your HS reflections.

Please complete this form ASAP, but no later than May 3rd.  

If you receive additional scholarships after submitting your questionnaire, please email the updates to afocht@wscloud.org so that we can try to add it into the program.  Please understand that information received after May 4th may not be able to be included.

Colleges and branches of the military ask for a Final Transcript.  We will send it to the place you listed in the previous question, but to make sure it arrives at the appropriate location, please indicate the City and State for your intended college/ military branch.                                                                  Please be aware that if you took CC+ classes you will need to request your final CC+ transcript from the college to be sent directly to your intended school.  Instructions and links to do this are on the Counselor website. *
For senior recognition, please list the exact names of all of the scholarships you were offered and are accepting.  We also ask that you put in ( ) the amount you received per year for each scholarship.  Please note the $ amount will not be included in the program, but is needed to be able to report how much money your class received in scholarships as a whole.
As a part of the graduation ceremony, you are able to choose a diploma presenter. This person must be a current or former Worthington Schools teacher, administrator, counselor, coach, or other district staff member. Who is your first choice (first and last name)? *
