Complete this form to request up to 5 books for check out.  Keep books up to 2 weeks and return when you pick up your next selections.  

Mrs. Nance and Mrs. Sykes will look at your requests and try to pick books you will love.  Use the box at the end to request a specific series or topic not listed.

If you want to request specific books, you can go to your Alief HUB-> Destiny Discover-> place up to 5 books on hold.  You can skip most of the google form boxes if you do this.  We will still need name, grade, ID, and contact phone number.  

Read the books you like and just return them in 2 weeks.  We will have curbside pick up during the school day when it is convenient for your family.  Make sure you return any books you currently have checked out when you come to pick up your new ones.

Worried because you can't find your book from last school year? It is OK.  If you have misplaced a book from last year, you can still check out books. Try your best to find them , and if you do, just return it.
