Puurrrfect Paws Rescue reserves the right to accept or deny any applicant or application based on our
requirements for adoption and the needs of the cat or kitten being applied for. Completion of the
application does not guarantee approval.

You must be at least 18 years old to be eligible to be adopt.  No Exceptions.
You must agree to an in home visit prior to approval for adoption.
You will be required to pay the adoption fee which covers vetting such as spay/neuter, rabies and 1FVRCP vaccinations

If the cat must be relinquished for any reason by the prospective caregiver, s/he MUST NOT turn the cat over to a humane society, shelter or person, but must return the cat to the Adoptee. If the phone number and/or address for the original caregiver is no longer operative,the caregiver will make a good faith effort to locate and contact the original caregiver. *

By signing this form, I/we acknowledge that all information on this form is true and correct. 
I/we understand that any misrepresentation of fact may result in  Puurrrfect Paws Rescue refusing adoption privileges to me/us. If my/our request for adoption is approved and later Puurrrfect Paws Rescue discovers the above information is not true or correct Puurrrfect Paws Rescue reserves the right to remove the adopted cat from my home/farm. 
I/we understand that cats adopted from Puurrrfect Paws Rescue are not permitted to be
declawed under any circumstances.  
I/we understand that cats adopted from Puurrrfect Paws Rescue are not permitted to be let
outside under any circumstances. Cats must be kept inside at all times.
By my taking possession of the animal, I release Puurrrfect Paws Rescue from and and all claims, demands, damages, actions, causes, costs, losses, and expenses arising from, related to, or connected with the animal, it's health, training, temperament, or intended uses. 
Puurrrfect Paws Rescue makes no representation or warranties, express or implied, with respect to the herein described animal's health, temperament, or training.
 I understand that failure to perform the foregoing agreement will constitute a breach of contract. In the event of any such breach of contract, I authorize the original caregiver to reclaim possession of the adopted cat. *
