Dear Parents/Carers

Term 1 commences on Monday, 1st February. To help us organise bus transport for students to/from school, we are asking ALL families to complete this form whether your child is catching school buses or being transported by car.

Please take a few moments to fill in this form and submit it to us by no later than next Wednesday, 27th January.

If you have more than one child at Sacred Heart and they have different transport arrangements could you complete separate forms for each child.

If you have any questions or specific instructions, please use the section at the end of this form.

A REMINDER THAT OUR SCHOOL BUS POLICY REQUIRES THAT ALL CHILDREN UP TO AND INCLUDING GRADE 4 ARE DROPPED OFF / COLLECTED BY A PARENT/GUARDIAN AT THE BUS STOP.
