CELEBRATE YOUR CHILD’S BIRTHDAY AT JAMS IN LIGHTS!
As a fundraiser, the John Adams Middle School PTO will be putting children’s birthdays on the school sign for a fee of $15.00 for two weeks. PTO Members receive a discount – current members pay $10.
Please submit your form to reserve your space. It is on a first come, first serve basis!
All proceeds help fund JAMS PTO-sponsored school events and activities! Thanks for your participation!
Please note:
The deadline to submit birthday requests is the end of each month.
We will not be accepting birthday requests for the months of July and August.
Only the child’s first name, followed by the first initial of their last name, will be placed on the sign. If your child has a nickname and you would prefer to display the nickname, please indicate below.
Please complete the bottom portion of this sheet and enclose it with cash, check or pay via Paypal. Please give us at least two week’s notice, or we cannot guarantee your child’s birthday will appear as requested.
1. Cash- Send exact cash with your child to their homeroom teacher, write your child’s and grade on a sealed envelope, labeled for “ JAMS PTO: Birthday Sign”
2. Check- Send a check with your child to their homeroom teacher. Make checks payable to 'JAMS PTO' and write your child's name and grade + 'JAMS PTO: Birthday Sign' on the envelope.
3. PayPal - Payable to: jamspto@gmail.com. Write your child’s name and grade & ‘Birthday Sign’ in the comment field marked.
