Please fill out this form 2 weeks in advance to request the SGA credit card. You can request the SGA Credit Card for a two-hour time slot between 9am and 4pm, Monday-Friday.

The credit card CANNOT be used for purchasing gift cards, anything on Amazon, or personal purchases. Please reach out to Bella Zaccardi (ir.zaccardi@assumption.edu) to purchase anything from Amazon and Sara Swillo Muckian (ss.muckian@assumption.edu) to purchase any type of gift card.

Please make sure that you do not pay sales tax. Student Activities or SGA will provide you with the form for tax exemption.

We ask that you please get a copy of your receipt and write your name, as well as your club/organization name at the top of your receipt upon turning it in to Student Activities in Hagan.
